What is meant by Care planning?

Care planning refers to the structured process in which the individual care needs of a patient are identified and a detailed care plan is developed to meet those needs. This plan includes assessing the patient's condition, setting care goals, planning and implementing specific care interventions, and regularly reviewing and adjusting the plan.
